Elbow pain sounds like a minor thing until it starts showing up in everything you do. Gripping a mountain bike handlebar on a descent at Pisgah, throwing a disc at a game in Carrier Park, typing through a long workday, even picking something up off the counter becomes something you are aware of and managing around. It tends to start as a specific ache after one activity and slowly expand until it is just always there.
The reason elbow pain becomes chronic for so many people is simple: the tendon gets treated but the neck, which supplies the nerves that control the whole forearm, never gets checked. ● Tennis Elbow (Outer Elbow Pain)
Pain on the outside of the elbow that gets worse when you grip, lift, or extend your wrist. Despite the name, most people who get this have never played tennis. It is far more common in people who do repetitive work with their hands, including keyboard users, tradespeople, and mountain bikers gripping handlebars on rough terrain. The tendons attaching the forearm extensor muscles to the outer elbow become overloaded and irritated over time.
● Golfer's Elbow (Inner Elbow Pain)
Pain on the inside of the elbow that gets worse when you grip and flex the wrist. Common in climbers at Rumbling Bald, paddlers on the French Broad River, and anyone who does a lot of pulling, gripping, or lifting with the hands. The forearm flexor tendons are the main tissue involved, and the pain sits at the bony bump on the inside of the elbow.
Did you know? The nerves controlling the forearm and hand come from the C6, C7, and C8 levels in your cervical spine. When those levels are restricted or irritated, the nerve supply to the forearm becomes sensitized, making elbow tendons more reactive and painful than they would otherwise be. Treating the elbow without checking the neck often produces incomplete results.
Three things consistently prevent elbow pain from fully resolving:
● Cervical nerve involvement at C6, C7, or C8 that keeps the nerve supply to the forearm in a sensitized, reactive state
● Restricted movement in the elbow or wrist joints that alters how load travels through the forearm with every grip and extension
● Chronic muscle tightness in the forearm extensors or flexors that keeps constant tension on the tendon attachment even when you are not doing anything
Rest reduces the load temporarily but does not fix any of these three things. That is why the pain keeps coming back after a break.
