Plantar fasciitis is one of the most common things Dr. Mark treats, and also one of the most reliably undertreated. Patients arrive having already tried the calf stretches, the frozen water bottle, the night splint, sometimes a cortisone shot. Some of those things helped for a while. None of them solved it.

The frustrating truth about plantar fasciitis treatment is that most of it is aimed at calming symptoms rather than correcting the underlying problem. Whether you are a trail runner in Pisgah, a hiker on the Black Mountain Crest Trail, or someone who works long shifts on their feet, the pattern is always the same: it calms down, you go back to your normal routine, and it comes right back.

What Plantar Fasciitis Actually Is

The plantar fascia is a thick band of tissue that runs along the bottom of your foot from the heel bone to the base of your toes. Its job is to support the arch and absorb impact when you walk or run. When it gets placed under too much stress, tiny tears develop where it attaches to the heel bone, the tissue becomes inflamed and thickened, and you get that sharp stabbing pain the moment you stand up after any rest.

The pain is usually worst with your first steps in the morning or after sitting for a while. It often loosens up a bit as you move around, which is deceiving because people think they are getting better. Then they do a long hike in the Pisgah or stand through a full shift, and it flares right back up again. That is not improvement. That is the same problem playing out on the same schedule every day.

That kind of foot loading, combined with the leg fatigue that builds on longer trails, is one of the fastest ways to develop restricted ankle dorsiflexion, one of the most consistent mechanical drivers of plantar fasciitis.

What Is Actually Keeping It From Healing

The three things that consistently prevent plantar fasciitis from fully resolving are:

Restricted ankle dorsiflexion, meaning the ankle does not bend forward far enough when you walk or run, so the plantar fascia has to stretch further than it should with every step to compensate

Poor foot and midfoot joint mobility, where specific joints in the foot are not moving properly and are shifting abnormal load to the heel attachment

Tight calf and Achilles tissue that limits how freely the ankle can move and keeps constant tension on the fascia from above

Stretching the calf addresses the tightness but does not restore the joint mobility. A night splint prevents shortening overnight but does not change how the foot loads during the day. Neither gets to the root of it.

How Dr. Mark Treats Plantar Fasciitis

Dr. Mark assesses the specific joints in the foot, midfoot, and ankle that are contributing to abnormal load distribution, checks first metatarsal mobility and big toe extension range, and evaluates calf and Achilles flexibility. Treatment combines chiropractic adjustments to the restricted foot and ankle joints to restore proper mechanics, instrument-assisted soft tissue work on the plantar fascia to break down the fibrous tissue that builds up with chronic irritation and trigger real healing, and eccentric calf exercises that have the strongest evidence base for reversing plantar fascia degeneration. Most patients notice meaningful improvement within the first few visits, and the goal is always to correct the mechanics so normal activity does not keep recreating the problem.
