Shoulder pain is one of the most common things Dr. Mark treats, and one of the most frequently mismanaged. Patients come in who have already had the shoulder treated, sometimes multiple times, with only partial improvement. The reason is almost always the same: the cervical spine, which supplies all of the nerves controlling the shoulder, arm, and hand, was never evaluated alongside the shoulder itself. Half the picture was treated while the other half was ignored.

Whether you are a climber at Looking Glass Rock, a paddler on the French Broad, a remote worker holding your arms at keyboard height for eight hours a day, or someone who woke up with the shoulder that just does not feel right, the starting point is always figuring out whether the problem is coming from the shoulder joint, the neck, or both. That answer determines everything about how care should proceed.

Shoulder or Neck: How Do You Tell?

True shoulder joint problems tend to produce pain directly in the shoulder with specific movements, especially reaching overhead, rotating the arm outward, or reaching behind the back. There is often a catch or limitation with those movements, and the pain stays fairly local to the joint itself.

Cervical spine involvement feels different. The pain might be in the shoulder area, but it can also spread into the upper arm, down toward the elbow, or feel like a deep aching that is hard to pinpoint. It is often accompanied by neck stiffness, and may include tingling or numbness into the arm. These are signs that a nerve root in the neck is involved, which requires a completely different treatment approach than a shoulder impingement.

Did you know? The shoulder is the most mobile joint in the human body. It can move in more directions than any other joint, which is exactly why it is also the most unstable and most prone to injury. That mobility is held together almost entirely by soft tissue, making the surrounding muscles, tendons, and the nerve supply from the cervical spine critically important to shoulder health.

Common Shoulder Conditions Dr. Mark Treats

Shoulder pain rarely has a single straightforward cause, which is why the same complaint can look very different from one patient to the next. Some cases stem from the tendons and bursa inside the joint itself. Others are driven by nerve irritation in the neck, poor mechanics in the mid-back, or weakness in the muscles that stabilize the blade. At Blue Ridge Chiropractic, the most common presentations we see include:

Rotator cuff tendinopathy, irritation and degeneration in the tendons that stabilize the shoulder joint, very common in overhead athletes and climbers

Shoulder impingement, where the tendons or bursa get pinched between the bones of the shoulder with lifting and overhead movements

Cervical radiculopathy into the shoulder, where a compressed nerve root in the neck refers pain, weakness, or tingling into the shoulder and arm

AC joint irritation, soreness at the top of the shoulder where the collarbone meets the shoulder blade, common after falls or heavy carrying

Frozen shoulder, a gradual stiffening of the shoulder capsule that limits movement in all directions and tends to get worse without treatment

How Dr. Mark Evaluates and Treats Shoulder Pain

Dr. Mark assesses both the shoulder joint and the cervical spine before deciding where treatment should focus. That means checking shoulder range of motion and specific orthopedic tests for the rotator cuff, impingement, and labrum, and also evaluating the cervical nerve roots for any involvement at C5, C6, or C7, the levels most likely to refer into the shoulder. Treatment is then directed at wherever the actual problem is: shoulder joint adjustments and rotator cuff soft tissue work when the issue is local, cervical adjustments and nerve decompression work when the neck is driving it, or both when both are contributing. Most patients notice a clear improvement in range of motion and pain levels within a few visits.
